Le Blanc Consulting is open to working with new clients for this Program. Clients will be provided a well-rounded curriculum of activities to meet their individual needs. Purpose and Goals of Services
This Program is specifically designed to assist consumers on a one to one bases in their homes and within their local community such as Walmart, the bank, laundromat, library, doctor’s office and much more. Through empowering them to make positive decisions, clients become self-aware and gain the ability to demonstrate self-control. Consumers are provided Independent Support Services and address the underlying behaviors that cause them to not be independent after being aging out of the foster care system, having living in care homes or with relatives. Consumers learn to create a new positive role in society, while adjusting to living on their own. Often clients may need more assistance as a result clients are provided two instructors that will split their time with the client or work with them together to assure clients best interest.

Our program provides a direct assigned staff member to work on a daily basis with one particular clients. This program is offered to clients who need additional support and assistance on a day to day basis. This program ensures that the clients will be under constant supervision during their daily routine. This includes their individual jobs, homework, table work, ect.
One-to-one service is provided to those who qualify. To determine qualifications parents or clients must meet or speak with their respective Case Manager/Social Worker and they will be the only individuals who may approve this program and make the necessary arrangements. Independent Living Program Concept We teach that Success in Independent living is gained by the work the clients invest to becoming empowered to make healthy choices within their community. We encourage a client to embrace the roles that they identify with within their community as a leader, teacher, parent, partner, sibling and friend. We know that fulfilling all these roles can be a challenge for any client, and it often cannot be achieved alone, therefore we provide wrap around support. Our comprehensive independent living skills program will teach or provide one to one assistance to consumers under three phases (The initial phase, transition phase, & the implementation phase). |

Independent Living Skills Desired Outcome & Plan
Outcomes are specific and goal-related for each consumer and their family relationship. Support systems are encouraged and useful for consumer success. Similar to other service provider programs, a healthy community of support is critical for our clients’ success. Our clients can achieve and reach their full potential in the areas such as, independent living, judicial compliance, individual health and nutrition, community awareness and safety, career exploration, financial literacy, communication, money management.
Independent Living Skills Program: Entrance Criteria for Clients.
· Consumers who are age 18 years and older who have a primary diagnosis with a developmental delay
· Consumers with a full scale IQ in the mild to moderate range of intellectual functioning.
· Consumers that have or exhibit a broad range of Mental Health Disorders.
· Clients of who require developmental assistance and enhancement of social/recreational skills.
· Consumers that need improvement on personal hygiene, grooming and other daily living.
Independent Living Exit Criteria. The ultimate goal for the consumer is to successfully complete the program. However the Regional Center Case Manager will be informed and consulted prior to any decisions. An ID team meeting will be held prior to the client being terminated or graduating from the program. A 30 day notification will need to be provided before any such actions take place any consumer who exhibits aggressive and/or inappropriate behavior that represents a danger to self or others may be asked to exit the program. Consumers in violation of terms and agreements within policy of the Regional Center and LeBlanc Consulting may be asked to leave the program. Consumers that refuse services may be ask to leave the program. Consumers that abuse the program, or staff members may be asked to leave the program.
