Log In Instructions
So you are curious about how to log in, and you can't reach a information systems, a supervisor or manager to help you? The system has not had any problems logging anyone in for more than 7 years. As a result If you are having difficulties logging in but can log into social media sites then it could be that you are not simply following the instructions below and log yourself in.
"Log me in Please"
Follow these steps to login to our system.
If you are having login issues, please make sure you have the most recent password as it is changed every 3 months or immediately upon suspicions of a data breach.
1. LeBlancConsulting.net
2. Click LOGIN icon on the top right and click "Login" once you are in
3. Password: (provided by IT management)
4. Scroll down to Daily Progress Notes
5. Consultant Information
6. Go to Consultant Name
7. Click drop down. Locate your name, and click on Done.
8. Go to Client Learner Name, click drop down to right.
9. Find Client's Name, and click on Done.
10. Go to down to Day of Week, use drop down to select day.
11. Repeat for month.
12. Repeat for day (date).
13. Repeat for year.
14. Ratio = 1 to 1
15. First time start, use drop down box
16. Repeat for First Finish Time.
17. Total hours: Add hours and click Dropbox.
18. CLIENT INFORMATION:
19. Use Dropbox for location.
20. Repeat for city.
21. Repeat for state.
22. Client phone number: ______________
23. Service information: Case manager,use DropBox to select.
24. Service provided is either ILS or Parenting
25. Objective, use Dropbox.
26. Goals addressed,
use Dropbox.
27. Materials used,
use Dropbox.
28. Take performance,
use Dropbox.
29. Scroll down to Daily Progress Notes
30. BIRP Notes
31. Submit Progress Note (Click on)
